Documentation
Individuals who have a documented need will be evaluated to receive services from the Office of Adaptive Services. Documentation must be completed by a professional and must be completed within the last three years for undergraduates and within the last five years for transfer or graduate students and /or diagnosis as an adult.
For individuals who are documenting a learning disability, the documentation must be a complete psychoeducational battery of tests. Examples of some of the tests that can be administered are:
- Wechsler Adult Intelligence Scale-Revised (WAIS-R)
- Woodcock-Johnson Psychoeducational Battery-Revised (WJ-R)
- Tests of Academic Skills (TASK)
Note: A licensed psychologist usually administers these tests.
For individuals who are documenting a physical or psychological disability, the documentation must be from a doctor or specialist who can document the extent of the disability and the ramifications of having such a disability.
IEP's and former teacher recommendations are helpful in eliciting services, but these types of documentation ARE NOT sufficient for primary documentation. In addition, if current documentation is too old and more up to date testing needs to be done, the Office of Adaptive Services will perform psychoeducational testing, with the cost of the testing being at the expense of the individual. The Guidelines for Documentation form will be helpful in choosing the appropriate documentation.
Process to Receive Services
- Documentation should accompany the FGCU application packet and sent to the admissions office. Documentation can also be sent to the Director of Adaptive Services. If documentation is sent to the Director of Adaptive Services, the documentation must be received at least 30 days prior to the start of classes.
- After documentation is received, the student must complete the Student Inquiry form and the Formal Request for Accommodations form. Both of these forms can be downloaded and sent in with the documentation. (All forms are also available in the office of Adaptive Services) The Director of Adaptive Services must have these forms in order to start the process of acquiring services.
- The student MUST meet with the Director of Adaptive Services within the first two weeks of the start of classes. It is recommended that the student make an appointment with the Director during the orientation period. It is not the responsibility of the Director to set up the initial meeting with the student. Discussions during this meeting will allow the Director to modify the accommodations and services if the need arises.
- If the status of a student's condition changes in any way that will affect his or her ability to succeed in the classroom, additional documentation will be needed. This documentation must be sent to the Director of Adaptive Services as soon as the change in condition is discovered.
- The student must meet with the Director at the beginning of EACH semester for evaluation and review of existing accommodations and academic performance.
*Delay of services due to late receipt of documentation and required forms are not the responsibility of the Office of Adaptive Services or the Director.
