*Due to the high volume of test scheduling and proctoring, the Test Schedule Forms will need to be picked up in the Adaptive Services office and returned, completed, within the 4 day timeframe. Please contact Adaptive Services if you are in need of a Test Schedule Form to be emailed to you.
