To register your organization for the year, purchase, and plan events , please download fill-out and return the necessary forms to the Office of Campus Involvement, Student Union room 215.
Register / Re-register a club
Please complete this Student Organization Registration form and attach a copy of the organization’s constitution. Once the form
has been returned to the Office of Campus Involvement please allow two days for processing. You
will be contacted by e-mail to confirm that your registration is complete.
Purchasing
Use this Purchase order Request form to make purchases from your organizations A&S or cash account. Please attach a quote from the vendor that includes a description of the merchandise or service, quantity of merchandise being purchased, and a total price. Please allow at least 10-14 business days for processing.
Event Planning
This Event Registration Form is in addition to making your reservation for space through
Campus Reservations. Your reservation is not officially confirmed until this form is submitted
and approved by the Director of Campus Involvement.
Please complete and submit the Sound and Lighting Equipment Request form to the Office of Campus Involvement at least 10 days prior to your event for approval. Your
organization must provide two able bodied volunteers to assist with setup and breakdown of the equipment (No special skills or
