Mid-term Progress Report

The SSS Staff is proactively working to ensure that participants are meeting “Satisfactory Academic Progress,” which is calculated both quantitatively and qualitatively. To facilitate this process, we are asking individual instructors to please assist the student by providing a midterm evaluation on the grid in the form that you can download below. This, in turn, will give SSS an opportunity to intercede on behalf of the student to review, discuss, and direct the student on matters that would affect their academic progress, academic standing, and/or student financial aid. Additionally, you, the instructor, may also find this process helpful in enhancing the student’s overall chance for academic success in your course.
